Out-of-combat health regeneration not always working

Sorry, I've found no way to reproduce this one. Out of combat health regeneration sometimes just turns off completely. Usually leads to a number of people all standing around on 30hp nervously eyeing the mobs :) I can confirm it's happened after I have died at least once. Can't remember if it has happened before the first death after logging in.
